The NCD Church Profile is a church health assessment report based on 30 surveys, comprised of 90 questions each. Printed surveys are shipped to you by postal mail. You will have the selected participants fill out the surveys, fill also the pastor’s form, and send all the forms back to NCD America. You can also use an electronic version of the survey.
 
The Basic NCD Church profile shows the basic scores of eight qualities and provides follow-up information related to the minimum factor (the quality with the lowest score).
